The purpose of this project is to gather information that will hopefully give us a better idea of the lifespan we may be able to expect of our budgies. I would greatly appreciate it if you would take the time to fill out this form. Please fill out a seperate form for EACH budgie that has passed. Only fill out this form for a budgie, NO other species of bird please! Answer as many of the questions as possible in as much detail as possible.
